Medication Administration Procedures at School
- Students who need to carry an asthma inhaler or other emergency medication must present documentation from a physician and parent outlining the need to do so. Asthma Action Plan are available for such documentation.

Medication Administration Procedures at School
- All prescription and nonprescription medications must be provided in the original labeled container and accompanied by a medication administration request form. A parent may also write a request as long as it includes the name of medication, dosage, time and frequency along with the parent/guardian’s signature.
- All prescription and non-prescription drugs to be administered or kept at school for longer than 15 days must be accompanied by a written request signed and dated by the prescribing physician and the parent or guardian
requesting this service. - All prescription and non-prescription drugs to be administered at school for 15 days or less must be accompanied by a written request, signed and dated by a parent or legal guardian.
- All prescription medication and medical procedures must have a physician’s order with instructions and be renewed at the beginning of each school year.

Medication Reminders:
A new medication administration request will be required each school year or with any change in the previous medication administration request ( ie. a change in the original time interval or dosage amount to be given).
A parent note gives the school nurse permission to dispense the medication according to the label on the original container. The medication must be age appropriate.
All medication will need to be picked up by a parent/guardian before the last day of school or the medication will be discarded.
